The Management Board of Creepy Jar S.A. with its registered office in Warsaw (hereinafter "the Company"), with reference to the agenda of the Ordinary General Meeting of Shareholders of the Company convened on 7
June 2022, presented in the current report No. 12/2022 dated 12 May 2022, informs that the authorized shareholder of the Company, Mr. Krzysztof Kwiatek, proposed the following candidates for Members of the Supervisory Board of the Company:

1. Mr. Michał Paziewski (current Chairman of the Supervisory Board);

2. Mr. Artur Lebiedziński (current Member of the Supervisory Board);

3. Mr. Tomasz Likowski (current Member of the Supervisory Board);

4. Mr. Piotr Piskorz (current Member of the Supervisory Board).

All the above-mentioned candidates agreed to be candidates to the Company's Supervisory Board and submitted statements on meeting the criteria required by law for candidates for members of supervisory boards, including statements arising out of the principles of the Code of Best Practice for WSE Listed Companies 2021.
Acting in accordance with the Code of Best Practice for WSE Listed Companies 2021 and ensuring that Shareholders have an opportunity to learn about the candidates for members of the Supervisory Board of the Company, the Management Board of the Company hereby forwards, as an attachment to this report, the professional CVs of the candidates and the respective statements.
